    Shalini!!! Hi! Can you please inquire from the derma doctors how to combat millia on the cheek? Skin that is smooth everywhere else and NOT at all acne prone but has millia. I have gotten it electro-cauterised and needled before. But they do come back. So what is a girl 1)to do at home? and 2)WHY am I getting them in the first place?

      Hi Parul,
      Milia are small cysts formed in superficial layers of the skin either due to blocked oil or sweat glands ducts. One can get them due to either of the causes:
      1. Primary phenomenon: occur spontaneously without any cause.
      2. Secondary phenomenon, due to injury to skin. Injury might have been caused due to vigorous scrubbing, cosmetic procedures like microdermabrasion or laser resurfacing or photosensitivity or radiation therapy.
      3. Occlusive cosmetic creams or ointment like steroids.
      4. It can be part of certain medical syndromes like rombo syndrome, bazex syndrome or bullous disorders of skin, but very very rare.
      Yes, electrodesiccation by radiofrequency or laser or needle extraction are the treatment of choice once they occur. To prevent them from recurring use either retinoid cream (Retino A 0.025% cream) or alpha hydroxyl acid cream (Glyco A 12%) at night. Apply only a pea-nut size cream, not more than that. Also take proper sun protective measures like umbrella, scarf etc. Swap all thick creamy cosmetics and skin care products for water based lotion or gels.

    Hello Shalini! I just wanted to know what it takes to keep blackheads off my nose! I keep getting them and I’m sick of removing them. Also my friends tell methat if i don’t get them treated, i could develop pores on my face. is that true?

    • Blackheads are blocked oil glands ducts due to accumulation of dead skin and oxidized sebum. So treatment should aim at exfoliating the dead tissue and reducing the excess oil production by the oil glands.
      1. Use retinoid creams at night (tretinoin or retino-a 0.025%) or hydroxyl acid creams( Glyco –A 12% ) Just apply a peanut size regularly to begin with and after a month, just twice a week.
      2. Wash your face with salicylic acid based face wash (Speelac, Saslic DS face wash).
      Retinoids and salicylic acid, exfoliates dead skin as well as reduces oil production.
      3. If this doesn’t help, go for few sessions of salicylic peels done by a dermatologist. All it takes it 10 minutes per session done once in ten days. This will loosen the pores and then they can be easily extracted by a blackhead remover.
      4. Single session of mesobotox over the nose would keep the blackheads at bay for 6 to 8 months. Yes you read it right, botox not only erases wrinkles but also blocks oil gland production thereby removing blackheads.
      5. Scrub regularly to prevent them from recurring. Either use walnut scrub available OTC or make your own home made scrub by mixing either honey, brown sugar and green tea or by just rubbing it lemon sprinkled with salt. Applying egg white for 15 – 20 minutes also serves the purpose.
      Don’t worry. No visible pores will be left behind.

    • Hi Smitha,
      1. Be regular with sunprotective measures to prevent further darkening. We rely on sunscreens to give us complete sunprotection and that’s why we end up tanning. Sunscreens protect from sun only if applied 20 minutes before moving out in sun, reapplied every 2 to 3 hours and applied atleast 2mg/cm2 . Do anyone of us do that? So go for other sunprotective measures side by side. Use umbrella with a silver lining inside which doesn’t allow sun rays to pass through. Wear protective clothings like hats, scarves, glares etc.
      2. Get few sessions of chemical peels by a dermatologist. Retinoid and TCA peels done once in 3 weeks would help. It just takes ten minutes per session and are pretty safe. Peels would help pigmented/ tanned skin exfoliate faster to reveal new fresh skin underneath.
      3. Use Kojic cream in morning and Glyco –A cream at night for two months.
      4. One can also resort to homemade remedies like alovera gel, cucumber juice or just rubbing raw potato to reduce the tan. But one needs to do it atleast for two months to see the visible effect.

    I had painful cystic acne on my cheeks few months back, when they healed, they left reddish brown scars on my cheeks. I would like to know, how can I make those scars go away ? What creams or topical treatment should I take ?

    • Hi Kokil,
      In case, you only have brown pigmentation left behind use Kojic cream in morning and Melalong cream at night( only over dark spots). Along with it, few session of chemical peels (glycolic or retinoid peels) would give great results. They just takes ten minutes per session and you can get them done by your dermatologist.
      But if they have left behind depressed scars then creams are not much of a help. Rather few sessions of subcision, dermaroller, fractional radiofrequency (scarlet) or fractional co2 laser would help. Yes , all are safe procedure if done under an experienced hand.

    My forehead is little darker than face. Any Solution?

    • Hi ramandeep,
      1. Use sunscreen regularly to prevent further darkening.
      2. If in habbit of rubbing forehead, stop doing that. Also if you have long hair rubbing against forehead, it’s better to get them cut short for constant friction can lead to darkening
      3. Use Demelan in morning and Melalong cream at night. Use them regularly for two months then just twice a week for next few months.
      4. Get few sessions of chemical peels (glycolic, TCA, or retinoid) and microdermabration by your dermatologist. It’s just take ten minutes per session and needs to be repeated every fifteen days. It’s pretty safe and effective.
      5. Take vitamin C tablets (Tab. Limcee 500 mg every day).

    This is a fantastic initiative Shalini. I have oily skin mostly but my scalp is really dry and flakey. I also get reddish acne like spots on the scalp.

    Because of the constant shedding I have gotten into the habit of washing my hair daily. I hate it when I can’t, the flakes build up.

    I want the scalp to return to normal. Hope I can get a long term solution.

    • Hi Dela,
      There is no harm in washing your hair daily, just make sure you use a shampoo which doesn’t leave your scalp further dry. Use Sebamed anti dry revitalizing shampoo- 100% free from soap and other harmful chemicals. In case you have red colored spots, use Sebowash shampoo twice a week. Using yoghurt once a week , either apply an hour before washing or leave it overnight. If all this doesn’t help, consult a dermatologist.

    Hey shalini. I have one question. My face and hands are 2 times darker than my skin.. I don’t its tanning or pigmentation….. I suffered because i dint used to wear sunscreen earlier… Have tried many home remedies but all useless. Can you plz tell me if I can make my face and hands similar to rest of my body?? any treatments available?

    • Hi Mansi,
      Darkening of sun exposed areas like face and hands compared to rest of the skin nothing but tanning. Prolonged tanning over the years is indeed difficult to get rid of.
      1. Begin with what you forgot to do earlier i.e. be regular with sun protective regimen: use sunscreen .One needs to reapply them every two to three hours. Just sunscreens won’t give you 100% sunprotection .Use umbrella with silver lining inside, to get real sunprotection from it. Also use glares and protective clothing like scarfs.
      2. Use Demelan cream at night and Kojic cream in morning for two months regularly then twice a week for next few months.
      3. Go for 6 sessions of either TCA or retinoid peels done every three week by a dermatologist. It will lead to deep exfoliation of pigmented cell revealing new untanned skin underneath.
      4. Home made remedies like rubbing raw potato , cucumber juice or aloevera can also be tried but one need to do them for atleast two month to see some visible results.

    Hi Shalini,Ive a very unevn skin which include dark undereyes and darkness near mouth area…but the most troublesome thing is the pits on my cheeks.I had lot of acne when i was a teenager.Now there isnt any blmishes but deep pits or scars on face,which really looks bad.I was told to do a chemical peel,but i was wondring if there is any other alternative…

    • Hi sheileka,
      For the pits and scars neither creams nor chemical peels would help. Peel are too superficial a procedure. Scarring and pits are due to fibrous tissue development deep in the skin. So, one needs atleast four to six sessions of either 100% TCA cross, or fractional radiofrequency (Scarlet) / fractional ablative lasers like Co2 /Erbium :Yag along with subcision ( in subcision fibrous strands pulling the scarred skin downwards are broken down using a special needle). All the procedures are painless (slight discomfort may be there) since are done after numbing the area with an anesthetic cream. Yes they are safe if done with US FDA approved lasers and by an experienced hand. However a downtime of 2 to 4 days can be there in form of little redness, swelling or crusting.

      For pigmentation around mouth use vitamin C serum( Rejuglow C20 ) in morning and Melalong at night continuously for two months and then twice a week for maintenance. Also take oral vitamin B12 and vitamin C supplements.

      As far as under eye darkness is concerned: use Strea light eyes or Eyez gel twice a day for atleast 6 months. Also go for few sessions of chemical peels to reduce the pigmentation around eyes and mouth. Peels remove he superficial pigmented layer and help reveal fresh skin underneath.

    Nia Asks : Not just one….Super oily skin plus tanned really badly and lots of blemishes and dullness,please suggest a solution

    • Hi Nia,
      1. To control the oil:
      A: Use salicylic acid based facewash (Speelac or Saslic DS facewash) twice a day.
      B: Use retinoid creams at night (Tretinoin, Retino- A o.025% or Strea a15).
      2. Get few sessions of retinoid peels by a dermatologist. It takes ten minutes per session and is done at three weeks interval. It will only take care of oiliness but also blemishes and tanning for peels help in exfoliation of superficial dead skin containing pigmented cells.
      3. To prevent further tanning and dullness use sunprotective measures (sunscreens, umbrella, scarfs, hats etc.). For oily skin use gel based sunscreen like Suncote or Capital Solel by Vichy.
      4. Apply Kojic cream in morning and Melalong cream (only over spots) at night continuously for two months then twice a week for maintenance.
      5. As far as glow is concerned , follow the following tips:

      If the skin is well hydrated and surface smooth, it reflects light evenly, which is seen as radiance and glow on the skin. Contrary holds true too, if surface of skin has irregularities and it lacks moisture, no amount of creams can make it look radiant.
      1 Drink plenty of water to keep yourself hydrated.
      2. The top most layer of skin is dead formed every 6 to 8 weeks, which should be scrubbed of to reveal the fresh skin beneath. Best way to achieve this by getting a session of chemical peel by your dermatologist. Chemical peels are nothing but concentrated fruit acid extracts, which can easily eliminate your dead skin layers. It just takes ten minutes per session and is absolutely safe if done properly.
      3. For lack of moisture, make flax seeds a part of your regular diet. They are an abundant source of in Omega 3 fatty acids, which are responsible for that skin glow. Just crush them and either mix it with your flour or curd. Flaxseeds by themselves doesn’t have any taste. One can also opt for omega fatty acid capsules. Don’t worry, they are pretty safe.
      4. Hyaluronic acid is transparent gel like substance naturally present our skin, responsible for imbibing 10,000 times more water than its weight. With age, its starts depleting leaving your skin dull. To replenish the lost hyaluronic acid, get three sessions of Restylene vital, at monthly interval by your dermatologist.

          Retinoids definitely have a role in antiaging but that’s not it’s only role or prime role. It’s a medicine with multitude of functions. By helping in faster exfoliation of superficial layers it helps to reveal fresh skin underneath, that’s how helping in removing blemishes.
          And retinoids are an integral part of acne management considering the fact that they reduce oil production by oil glands.
          Infact, though they do have a role in collagen production but lasers and dermarollers are prime modalities for collagen production. Retinoids are just an adjuvant for this purpose.

              As I mentioned earlier, use Kojic cream in morning and Melalong at night. Melalong contains retinoids in it. But Melalong has to be used strictly at night only, just squeeze out a pea nut size cream, its enough for entire face,avoid applying around eyes and mouth region. Use for two months and then stop it. Not to be used more than two months.

    My basic skin query is pigmentation and dullness of the face.I have c-shaped pigmented area at the corners of the face and around my mouth which makes my all over facial skin appear darker.Also,my skin looks dull.Can you pls suggest something to get rid of this pigmentation and brighten up my facial skin.I use sunscreen when I step out but I feel that I still get tanned.(I use spf 50PA+++)
    Regarding,hair…these days I am having major hair fall and I want to stop the damage.My hair have become frizzy as well.
    Lastly,my nails don’t grow up and chip off very easily.
    Looking forward to solution of the 3 queries.

      For brittle nails,
      1. Take biotin supplements daily for two months. ( Tab. Xtraglow or Tab. Vighan forte )
      2. Use Si Nails varnish with Auriga international marketed by Palsons derma.
      3. Use the nail shades which are free from harmful chemicals. Buy lotus or MAC nail shades which specifically mentions toluene and DBP free.
      4. Use acetone free nail shade remover. SI nail remover by Auriga internatonal is acetone free.
      5. Use too much exposure to harsh chemicals, soaps and detergents.
      For frizzy hair:
      1. Shampoo: Use LOREAL Liss ultime oil incell shampoo or sulphate free shampoos like Kerastase chroma captive shampoo by L’oreal, Giovanni smooth as silk shampoo, Johnson’s and Johnsons baby shampoo, Body shop banana shampoo, Himalya herbal anti dandruff shampoo. . Sulphate has detergent like action, witch though leaves your hair squeaky clean but at the same time rob them of its natural oil too leaving them dry.

      2. Conditioner: Shampoos strip off the natural oils from hair leaving them dry and frizzy. So replenish lost moisture by using conditioners like – Tony and guy smoothening frizz curl conditioner or Schwarzkopf Gliss total repair conditioner.
      3. If this doesn’t help go for leave in conditioners or serums like- L’oreal professionel ultime serum. Its sulphate and silicone free.
      4. Go for hair spa for deep conditioning once a month or make your own hair spa by either applying mashed banana or yoghurt and honey mixed half an hour before washing.
      5. Have omega 3 rich diet – walnuts , flaxseeds, salmon fish. Omega 3 is best bet to replenish lost lusture.

      For hair fall

      Remember a scalp has 100,000 hair. Falling of 50 to 100 hair per day is considered normal. If hair fall hovers beyond this figure then one should to be cautious.
      one needs to find out the cause before going ahead with the treatment.

      1. Acute illness like fever, typhoid, hepatitis, surgery, psychological trauma etc. can lead to hair fall. Remember Hair fall IS not immediately but occurs after three months of the episode of illness.
      Treatment: It usually resolves own its own after the episode of illness ends. Just take high protein diet.
      2. Hormonal changes: Thyroid abnormalities, PCOD (polycystic ovarian disease) etc. Get the hormonal profile checked (TSH, T4, T3, free testosterone, LH, FSH etc.)
      Treatment: If any hormonal abnormality is detected, consult your physician hormonal therapy.
      3. Nutritional deficiency: Diet low in protein and essential vitamins like: iron, zinc, vitamin D etc. get the serum levels of iron and vitamin D3 checked.
      Treatment: If found low take supplements and diet rich in the same.
      4. Medications: Certain medicines might lead to hair fall. Antihypertensives (beta blockers), anticonvulsants, heavy metals, anticoagulants etc.
      Treatment: Consult your physician, see if after weighing risk: benefit ratio if he /she can provide some benefit.
      5. Androgenic alopecia: Male hormone ( testosterone) induced hair loss. The most common cause of hair fall in men indeed.
      Treatment: Apply Minoxidil solt. twice daily regularly, infact religiously . It will prevent further thinning and backward regression of the hair line.

      HAIR LOSS TREATMENT IN GENERAL
      1. Begin with diet rich in protein and essential minerals like biotin, iron, zinc etc. You can even take supplements (Cap. Androgen ,tab. xtraglow, tab. vighan forte etc.) to make up for the deficiency.
      2. Use Minoxidil solution (Hair 4 U Solt.) twice a day religiously and regularly. It might not be very effective in getting the lost hair back but is the treatment of choice to prevent further regression of the hair line backwards.
      3. Hair mesotherapy: To increase the density of hair, to get the lost hair back and to make the already existing hair thicker opt for mesotherapy. One needs to take 10 to 12 sessions at a good aesthetic center, initially at weekly interval and later at fortnightly interval where stem cells and other anti- hair loss solutions containing growth factors for hair are directly injected into the hair roots. You can also club it with three to six PRP (platlet rich plasma) sessions, done at monthly interval where your own blood is taken, growth factors separated from it and then reinjected in your hair roots. Hair mesotherapy is very safe and effective procedure.

      c shaped pigmentation favours the diagnosis of pigmentory demarcation lines. nothing much can be done about it but still consult a dermatologist first before jumping to conclusion.

    Thank you for the amazing platform.
    I have dry scalp. But there is no itchiness. But the dryness is prolonged and keeps recurring.
    The dryness is only in the scalp.

    Hair is not dry though. I have no split ends. The volume of the hair is pretty good. I apply oil and leave it overnight before washing. I wash my hair 3 times in a week.
    It would really helpful if there is a solution for my dry scalp.

      1. Use antidry shampoo like Sebamed antidry shampoo.
      2. You need omega 3 acid to combat dryness. It’s not made in our body but needs to consumed. Have salmon fish, walnuts and flaxseeds. Ground flaxseeds and mix them in flour to have them on regular bases.
      3. Keep yourself hydrated by drinking atleast 8 glasses of water every day.
      4. Oil the scalp once a week or fortnight. Coconut or olive oil applied after prior warming a little would be better as warmth leads to better penetration.
      5. Avoid hair bath with hot water. It would strip off natural oils from scalp leaving it dry.

      General tips for healthy hair:
      1. Avoid hot water wash.
      2. Cut down on chemical and heat exposure by reducing use of hair drier, colours etc.
      3. Sunprotection is must: use scarf or hat to cover up hair.
      4. Don’t comb wet hair. Wet hair is weakest and most prone to damage.

    hello i m 27 and suffering frm horrible dark circle wid sunken eyes…i hv used many under eye creams derma treatments .they prescribed me kojivit cream and some gave me luma glow(hydroquinone tretinoin fluocinolone acetonide ) cream.and also i hv tried 2session of peeling..bt no gain they r as always thr to haunt me..i knw its hereditry ..my mom has dark circles too..my Hb level is 10…my sleep is adequate 7-8 hrs..
    so plz can u suggest me anything that can lighten up them i knw i cant erase them bcoz they r hereditry..

    • True Neha, getting rid of hereditary under eye dark circles is tough, but that’s not the end of the road. Begin with using a concealer. Either go for Cernor XO under eye coverstick by Auriga international marketed by Palsons or concealers by Dermacolor (a product by Krylon cosmetics).
      Coming to the treatment, for sunken eyes single session of fillers might help. Only after examining you, one can give concrete opinion. Here a transparent gel like substance called hyaluronic acid, is injected to make for the volume loss.
      For removing pigmentation, peels is good option but just two sessions won’t suffice. One needs minimum of 6 session to see some visible results. As far as using lightening creams are considered, again one needs to use them for atleast 6 months to see some results. Try using Wunder eyes/Eyez 40 / Strea light eyes/ Rejuglow C20 cream etc. If peels and creams fail, one can consider getting Nd: Yag Q switched laser.
      Lastly, don’t rub your eyes too often, it breaks the blood vessels releasing iron and heamosidrin, which when gets deposited in skin, leads to further darkening of skin.
      So reconsult your dermatologist for the best possible treatment plan.

    1- I have a oily skin and due to that I have face lots of pimples and blackheads. I want to get rid of these pimples now and how to take care of my skin?

    2- Before my marriage my hair were thick and strong but after my marriage they get weak and thin. And from last three months hair fall has increased a lot what should I do? Some of hair get gray and some of them are white. Not even this I have seen my eyebrows also have 2 white hair. What is the reason for this?

    • Hi Bhawna,

      For acne at 35, one needs to rule the underlying hormonal cause very first. Get your free serum testosterone, DHEAS, FSH, LH levels done on second or third day of periods. In case they are deranged , one needs hormonal treatment for the same. In the meantime, start with salicylic acid based facewash twice a day, an antibiotic based cream in morning( benzoyl peroxide or clindamycin) and retinoid based creams at night( Epiduo or Tretinoin). For blackheads get them removed by a comedone extractor at a good aesthetic center.
      As far as thinning of your hair is considered, it’s probably androgenic alopecia. Unlike men, women don’t loose hair completely but end up with diffuse thinning of hair and widening of central partition. Use 2% minoxidil solution (Hair 4 U solution etc.) twice a day regularly. Take biotin supplements 5mg/day for two months (Tab. Xtraglo, cap. Andranagen) .Have protien rich diet for hair are largely made of proteins. Also have Indian goosberries (amla) and almonds regularly. Lastly get your iron levels checked (serum ferritin level), for iron deficiency is the most common cause of hair fall in Indian women. If found low take iron supplements. No harm in getting thyroid levels checked too, for their derangement too can lead to hair fall.

    my corners of the nose and mouth are a little darker than my original skin color.
    Is there a way to solve the darkness.

      1. Use vitamin C based creams in morning (Rejuglow C20 cream) and creams containing kojic acid, arbutin etc.( Kojic cream, Kojivit ) at night.
      2. Also moisturize the area regularly with your regular moisturizer.
      3. Have supplements containing vitamin C and B ( Cap. Becasule etc.).
